Renew a South Carolina DOT Medical Card

To renew a DOT medical card in South Carolina, begin with a new examination by an FMCSA-listed certified Medical Examiner. The medical-certification update is related to your commercial driver's license record, but it is not the same transaction as renewing the CDL itself.

The safest approach is to check the expiration date early, schedule before the last week, and arrive with the records your examiner may need.

1. Check the expiration date and your driving category

Start with the medical-certification expiration date on your CDL record or current paperwork. South Carolina CDL holders also self-certify the type of commerce in which they operate. Whether medical certification is required depends partly on that category.

2. Schedule with a certified Medical Examiner

Tell the office your expiration date when you call. Waiting until the final day leaves no room to obtain a treating-clinician letter, corrected form, or other documentation if the examiner needs it.

3. Bring complete information

Bring:

• A government-issued photo ID
• Your current medication list with doses
• Glasses, contacts, or hearing aids used while driving
• Relevant medical records and recent test results for ongoing conditions
• Any applicable FMCSA diabetes, vision, or medication form
• CPAP compliance information if your treating clinician or examiner has told you it is needed

4. Complete the examination

The Medical Examiner reviews your history and performs the checks recorded on Form MCSA-5875. A new certificate is not automatic. The result may be full-term certification, a shorter certification period, a need for more information, or a finding that the current standards are not met.

FMCSA permits certification for up to 24 months, but the examiner may choose a shorter period when monitoring is appropriate.

5. Verify the electronic update

Since June 23, 2025, certified Medical Examiners electronically submit CDL and CLP medical-certification information through FMCSA's National Registry for transmission to state licensing agencies. SCDMV states that manual submissions to its CDL Help Desk are no longer accepted.

Drivers should still keep the paperwork provided after the visit and use SCDMV's online status check to make sure the record is correct. Electronic reporting does not remove the driver's responsibility to maintain the right self-certification category and a current CDL record.

CDL renewal and medical renewal are different

When you renew or upgrade a South Carolina CDL, SCDMV may also require its own application, vision screening, fees, and Form DL-405A. Review the current SCDMV CDL renewal instructions before visiting the DMV.

A DOT exam updates medical qualification; it does not renew the license itself.
