How long do DOT medical cards last? An FMCSA Medical Examiner's Certificate may be valid for up to 24 months, but two years is a maximum—not a promise attached to every DOT physical.

The certified Medical Examiner chooses the expiration date after reviewing the driver's history, examination findings, treatment, and the applicable federal standards. Some drivers receive a shorter certification period so a condition can be monitored sooner.

Why a certificate may be shorter than two years

A shorter period can be appropriate when the examiner needs earlier follow-up for a health condition or finding. Examples of information that may affect the decision include blood pressure, treatment stability, medication effects, vision or hearing findings, diabetes care, sleep-disorder management, and specialist recommendations.

These examples are not automatic timelines. The Medical Examiner must make an individual decision under current FMCSA standards.

A shorter certificate is still a valid certificate

Drivers sometimes treat anything under two years as a failed examination. That is not accurate. A shorter certificate means the driver was found medically qualified for the period shown, subject to any restrictions or requirements documented by the examiner.

The expiration date matters. Plan the next examination from the date printed or recorded, not from the date you expected to receive.

What if the examiner needs more information?

The examiner may need a treating-clinician form, test result, medication explanation, or specialist record before making a final qualification decision. Bringing complete records is the best way to reduce avoidable delays.

Choose a date that leaves time to obtain any missing record before the current certification expires. How early to schedule depends on appointment availability and the complexity of your medical history, but the last business day is a risky plan.

Certification and CDL expiration are separate

The medical-certificate expiration date and the CDL expiration date are not interchangeable. A new DOT examination does not renew the driver's license. SCDMV handles license applications, self-certification categories, testing, and fees.
